Our patient, a 50 something female, had been fitted with porcelain veneers approximately twenty years ago. The veneers shown in this before photo were worn 21 years. At the original time of treatment, the use of veneer laminates was practically still experimental.... compared to current treatment standards.The superb longevity of the veneers did impact the gingival tissues somewhat. Some of her gum tissue receded. There was also evidence of veneer discoloration between several of her anterior teeth.
Decay was found to be present also in some of the tooth structures, which further compromised certain strength and functional characteristics of the affected teeth. The second photo reveals the individual tooth structures after the veneers were removed.Old, in-between-the teeth restorations were taken out, decay was excavated and the teeth were prepared for final, classical crown restorations. Functional and CosmeticAfter discussing a variety of treatment plans and the use of different types of dental components for each choice of treatment, our patient decided to focus equally on dental function and esthetics.It then became a choice of materials and picking the most estethic material capable of a good bond strength, durability, tissue tolerance, marginal adaptaion and resistance to fracture. A tradtional porcelain to metal crown, including one with porcelain margins was considered, however due to her age, considerations of longevity, cosmetics, tissue recession (to avoid that dark line up at the gum line), and the inherent risks of porcelain fracture, premium Procera crowns were selected as the material of choice.
